Turning Community Health Initiatives Into Revenue
Proactive health initiatives, in the form of Community Health Improvement (CHI) programs have long been a significant focus for hospitals, particularly those with religious, government, or community sponsorship and mission focus. They also serve as a basis for public relations and image building, for all kinds of hospitals, and for justifying and preserving tax-exempt status for nonprofit hospitals. They may help overcome recent scandals regarding the "overcharging" of un- and under-insured patients. But, like all hospital activities, they are under constant "attack" by payors who keep grinding down how much they contribute to hospital financing, and leave increasingly less margin to use for CHI efforts.

As Medicare, Medicaid and commercial health insurance all cut payment, wherever they can, and as the numbers of uninsured and amounts of payments required from insured patients with greater share of the payment burden both increase, the ability of hospitals to continue to fund CHI programs is constantly pressured. Unless endowments, fund-raising, grant support, and volunteer efforts can make up for declines in operating margins, the future of CHI is certainly precarious.
